- kind of hard to explain, but here goes:
- The Doctor was trapped in a device called a Confession Dial. In order to escape it, he had to confess different things about himself. When he would get to a certain point where he was cut off by a wall made of something harder than diamond and he couldn't reveal any more of his deepest secrets, he would end up being nearly killed by a terrifying monster, crawling back to the room he started in, and having to sacrifice himself to start things over again via a machine that brought a copy of himself from when he first was trapped back into the Confession Dial. Every time he failed though, he would punch the wall as many times as he could, which, over the course of two billion years and millions and millions of run throughs of the same events happening over and over again, eventually broke through the wall and let him escape.
Hope that makes some sense/is somewhat accurate.
